Energize Bridgewater Feedback Form
You are invited to complete this form because you recently took part in the Energize Bridgewater Home Upgrades program. We would like to hear about your experience. The answers you provide will help the Energize Bridgewater team better understand your experience. We will use your input to make program improvements for the benefit of others.
Unless you provide your name, the survey is anonymous. Your participation is voluntary. Whether or not you decide to participate will not have an impact on the services you receive through the Town of Bridgewater or the Energize Bridgewater program. The responses you give will also not have an impact on any of these services.
Your responses are confidential. Data will be securely stored by the Town of Bridgewater and will be kept for a period of up to 7 years. Only Town of Bridgewater staff working for the Energize Bridgewater program will have access to the data. No material that could personally identify you will be used in any reports, summaries, or publications coming from this survey.
